Prison Discharge Planner / Corrections Social Worker / Correctional (No License Required) Looking of a Social Worker to work directly with prisoners completing the assessments and assisting with community resource coordination (food programs, housing programs, drug rehab, medical and similar) once released.
TRAINING:
3 weeks of required Training to be completed in Elizabethtown, PA.
Travel will be reimbursed Overnight stay will be provided to stay during the week, Saturday and Sunday will be at home.
Report time on Monday 11am, release time on Friday 12pm to give time for driving.

00 Job Duties:
Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment.
Assist with referrals in the community, including setting up and scheduling appointments for medication and treatment.
Ensure needs, benefits, and services are obtained in accordance with assigned individuals' treatment plans.
Track and update spreadsheet/database on everyone released from the facility Meet with reentrants with positive urine drug screens to assess and refer to treatment.
Complete assessments as required (Initial, 6 months, discharge).
Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.
Requirements:
Bachelor's in Social Worker or Human Service field required.
Knowledge and experience in Case Management, Discharge Planning and Community Resources.
Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families.
Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.
